病変 is the most general and common medical term for 'lesion' in imaging and pathology. 損傷 emphasizes injury or damage, often from trauma. 病巣 is a focus of disease, used in infectious or inflammatory contexts.

In everyday Japanese, people rarely say '病変' unless in a medical context. Instead, they describe the specific symptom: 発疹 (rash), 傷 (wound), できもの (bump), etc. Using '病変' casually may sound overly clinical.
